Information you provide
Forms collect contact details, service preferences and the clinical context you choose to provide. Accounts store your name, email and password hash. Do not submit emergency information or unnecessary medical details.
How the website uses it
Information is used to review requests, coordinate appointments, maintain patient records and provide account access. Requests are stored in the website database. Receipts and account-recovery messages are queued for email delivery; a saved request is not an appointment confirmation.
Access and tracking
Administrators manage requests and access. Medical professionals access patients linked to their assigned requests. Keep your tracking code private: it allows a request status check. Guest requests can be linked to an account using the matching email and tracking code.
Cookies
The application uses session and security cookies for sign-in and protection against forged requests. Your language choice is stored in this browser.
Questions and requests
Contact info@qaziim.kz to ask about your information, corrections or deletion. Do not send passwords or tracking codes in your initial message. Retention periods, email-provider arrangements and the institution’s approved privacy policy require confirmation from QAZIIM; this page describes the website implementation and does not promise a deletion deadline.
